How the application works

To apply, you'll need to complete the client form yourself, then ask your doctor or nurse practitioner to complete a separate verification form. Both must be submitted before your application is reviewed.

STEP 2 - YOUR DOCTOR

Healthcare Worker Form

Your referring doctor, nurse practitioner, or specialist must confirm your diagnosis and support your application.

  • Confirms patient eligibility

  • Recommends suitable programs

  • Completed by a licensed provider

  • Takes about 3 minutes

Please note: Your application will NOT be reviewed until both forms have been submitted. After completing Step 1, please share the healthcare worker form link with your doctor or clinic.
